IMPORTANT HEALTH INFORMATION FOR SWMHS STUDENTS 2014-15

Asthma Action Plan completed by their physician and signed by a
parent/guardian. A new Asthma Action Plan must be submitted for each new
school year. Forms are available in the Nurses’ Office. Please be sure
that the physician checks the spot allowing self-administration if
. Any student taking medication in school must have a medication form
completed by a physician and signed by a parent/guardian. This includes
all prescription and all over-the counter medications. Medication forms
are available in the Nurses’ Office. New forms must be submitted each
school year, and the medication, in the original container, should be
brought to school by a parent.

parents/responsible designee can be contacted in case of illness or
